The efficacy of nateglinide, a potent oral antidiabetic agent that is rapidly absorbed and undergoes extensive first-pass metabolism, is associated with cytochrome P450 (CYP) 2C19 activity in Caucasians. Whether such association can be found in other ethnic groups is unknown. We therefore investigated whether CYP2C19 activity is associated with the efficacy of nateglinide in the Japanese population. Type 2 diabetic patients (n = 34) with HbA1c values of > or = 7.0% (n = 9) and or =70% urinary 5-hydroxymethyl-2-pyridone-carboxylic acid (5-HMG) excretion and those without a secretory defect of 5-hydroxytryptophan (5-HTP) were classified as extensive metabolizers (EMs) and poor metabolizers (PMs), respectively. As a result, eight subjects were classified as EM and 26 as PM with a significantly higher proportion of PMs than that of reported in other Caucasians. Nateglinide produced a progressive lowering of HbA1c and a dose-dependent increase in the insulin secretory responses in all patients. The baseline HbA1c and the doses of nateglinide that induced 1.0 mmol/L increments in plasma glucose were significantly lower (6.8 +/- 0.2% vs. 7.4 +/- 0.2% and 100 +/- 9 mg/day vs. 151 +/- 8 mg/day, respectively) in PMs than those in EMs. The increment of plasma glucose in response to nateglinide in PMs was 42% less than that in EMs (P = 0.0477). The population of Japanese is likely to have a polymorphism of CYP2C19 that confers lower nateglinide efficacy in insulin secreting cells than that of other ethnic groups.Q: 0b46394aab
